Your CURRENT doctor / nurse practioner.

We make contact. Human-to-Human.
We contact the primary care providers involved. We confirm they are willing to make a swap. We get final agreement from everyone involved.
We call providers directly, making sure that everyone is happy.
We advocate directly on your behalf: this is not an algorithm.
Swap
You get a new primary care provider. Closer to home.
If everyone is happy, and everyone is in agreement, you pay the fee, and a swap is made.
You are under no obligation to make a swap.
Your information is never revealed to any other patients.
